Most military record requests received at NPRC are for information from: the Official Military Personnel Files (OMPFs) of service veterans; clinical records (inpatient) of military personnel while on active duty; and medical treatment records (inpatient and outpatient) of military and non-military personnel who received treatment at U.S. Military Treatment Facilities (MTFs) worldwide.
Official Military Personnel Files (OMPF) which contains the Active Duty Health Record (also known as Service Treatment Record, which could include outpatient, dental, and mental health treatment information).
Clinical and Medical Treatment Record include:
- Clinical records (hospital inpatient) created for military personnel while on active duty.
- Medical Treatment records created for military retirees, dependents, and other non-military personnel that received treatment at MTFs (includes inpatient or outpatient).
Retirement of Military Treatment Facilities (MTFs) Medical Records at NPRC
NPRC stores retired Military Treatment Facility (MTF) clinical records (hospital inpatient) and medical treatment records (inpatient and outpatient) for the Army, Navy, Air Force, and the Defense Health Agency (DHA). MTF medical records retired to NPRC from 2002 onward are electronically indexed in the NPRC Medical Registry System (MRS). In order to retire these records, MTF PAD POC personnel must have access to the ARCIS/MRS MTF Customer Web Portal.
