The Controlled Unclassified Information Executive Agent (CUI EA) develops resources for Executive branch departments and agencies that handle unclassified information that requires safeguarding or dissemination controls, pursuant to and consistent with applicable law, regulations, and government-wide policies.

Agencies may use a coversheet to identify CUI, alert observers that CUI is present from a distance, and serve as a shield to protect CUI from inadvertent disclosure. If an agency chooses to use a coversheet, it must use CUI EA-approved coversheet Standard Form 901.

NOTE: Standard Form 901 replaces forms OF901, OF902, and OF903, rescinded on December 14, 2018. Agencies may continue to use Forms OF901, OF902, and OF903 until existing supplies have been depleted.
