Item Approval Request Form: Special Media

The Item Approval Request List Form can be obtained in a number of ways:

2. NA Form 14110, Item Approval Request Lists (IARL), (Microsoft Word ".doc" format), complete it, and then e-mail it to us at  mopix@nara.gov.

   The attached file--NA Form 14110, Item Approval Request Lists (IARL)-- was created using MSWord 2003.
   
   When filling out the form:

   • Please provide complete information on the IARL for boxes "A" (customer/researcher information), "B1" (control and reel numbers--use the number that appears in the "Local Identifier field"), "B2" (subject), and "B3" (order copy format, this is the copy you want delivered to you).

   • Each reel must be listed separately.

   • You may order up to 12 reels per order and you may submit two IARL per week.

   • Please return completed IARL as an e-mail attachment or by mail. You may make multiple copies of this form.
